Pre-emptive Delivery of Pain Specific Local Anaesthetic (QX-CAP) to Prevent Endodontic Postoperative Pain

Outline of Annual Research Achievements

The aim of this study was to investigate the possibility of using the combination of QX314 & capsaicin (QX-CAP) in preventing postoperative neuropathic like pain after endodontic treatment. Therefore we need to establish a model of endodontic treatment in rats. In the last year, in a pilot study, we tried to establish a method to make a rat model with endodontic treatment procedure. We tried with pulp exposure of the rat`s lower central incisor using dental dril and followed by repeated stimulation of dental pulp using a endodontic file to mimic the endodontic procedure. We checked the c-Fos expression in the spinal trigeminal sub-nucleus caudalis (Vc). The c-Fos expression was increased in the trigeminal sub-nucleus caudalis, two hours following the repeated stimulation of dental pulp using a endodontic file that mimicked the endodontic procedure. We than investigated whether prior infiltration ofthe combination of QX314 & capsaicin (QX-CAP), in the oral mucosa labial side of the incisor reduced the c-Fos expression that was increased due to the endodontic procedure. We observed that pre-emptive delivery of the QX-CAP reduced the increased c-Fos expression in the spinal trigeminal sub-nucleus caudalis.

Reason

A rat model was prepared to mimic the endodontic procedure. In the pilot study, c-Fos expression increased by the mimicked endodontic procedure as expected. The preemptive delivery of the combination of QX-CAP reduced the increased c-Fos expression in the spinal trigeminal sub-nucleus caudalis in the pilot study.

Strategy for Future Research Activity

We need to elaborate the results of the pilot study by increasing the number of rats and investigate that how much c-Fos expression in the Vc will be reduced by preemptive delivery of the QX-CAP. We will check the c-Fos expression in the Vc following pre-emptive delivery of QX314 only. We will also check the c-Fos expression in the Vc following pre-emptive delivery of capsaicin only. We also need to investigate by doing same procedure in other groups of rats, like sham operated group, naive group, following pre-emptive delivery of lidocaine or saline.

Causes of Carryover

The small remaining ammount will be used to buy research items in the next year.
