Pre-emptive Delivery of Pain Specific Local Anaesthetic (QX-CAP) to Prevent Endodontic Postoperative Pain

Outline of Annual Research Achievements |
We have been investigating the possibility of using the combination of QX314 & capsaicin (QX-CAP) in reducing postoperative pain after endodontic treatment. Making animal model to mimic endodontic treatment: We exposed the dental pulp of the rat`s lower central incisor using dental drill. We then repeatedly stimulate the dental pulp using an endodontic file to mimic the endodontic treatment procedure. Neuronal activation by the mimicked endodontic procedure: To understand the neuronal activation, we checked the c-Fos (a marker for neuronal activation) expression in the spinal trigeminal sub-nucleus caudalis (Vc). c-Fos expression increased in the Vc, two hours following the endodontic procedure. The number of neurons with c-Fos expression was more in rats with endodontic procedure than sham rats (needle prick and drill on the central incisor without exposing the pulp) and naive rats. Effect of pre-emptive delivery of local anesthetic on neuronal activation: We are now investigating the effect of prior application local anesthetics on neuronal activation to the endodontic procedure. We are checking the effect of local infiltration QX-CAP in the gingiva near the central incisor and the effect of infiltration QX-CAP near the mandibular division of the trigeminal nerve on the neuronal activation. We are also checking the effect of local infiltration of lidocaine (a commonly use local anesthetic) on the neuronal activation. In preliminary observation, the number of neurons with c-Fos expression decreased when QX-CAP solution and lidocaine was used as mandibular nerve block.

Reason
1. Animal model mimicking endodontic procedure has been made. 2. c-Fos expression increased in the spinal trigeminal sub-nucleus caudalis (Vc) regions by the mimicked endodontic procedure. The number of neurons with c-Fos expression was more in rats with endodontic procedure than naive/sham rats. 3. Blocking of mandibular nerve by QX-CAP and lidocaine decreased the number of neurons with c-Fos expression.

Strategy for Future Research Activity |
1. Increasing the number of animals and prepare the results of c-Fos expression quantitatively. 2. Checking the effect of infiltration of QX-314 near the mandibular nerve on the neuronal activation. 3. Checking the effect of infiltration of capsaicin only near the mandibular nerve on the neuronal activation.
