Outline of Annual Research Achievements

肥厚性硬膜炎(Hypertrophic pachymeningitis; HP)は頭痛や脳神経麻痺をきたす原因不明の神経難病であり、私たちが行った全国疫学調査では抗好中球細胞質抗体関連が38%、IgG4関連疾患が12%を占めていた。私たちはヒトIgG4関連疾患のモデル動物であるLATY136Fマウスにおいて、硬膜の炎症と線維性肥厚が起こることを発見した。硬膜病変ではT細胞、B細胞、マクロファージ、形質細胞、好中球と多彩な炎症細胞が浸潤し、IgG1(ヒトIgG4に対応)陽性細胞も多数みとめられた。さらに、HE染色では硬膜の顕著な炎症細胞浸潤に伴い小静脈の内腔が閉塞する閉塞性静脈炎(obliterative phlebitis)の所見もみとめられた。Masson’s-Trichrome染色ではLATマウス硬膜の線維化をみとめ、一部では花筵状線維化(storiform fibrosis)を呈していた。また、collagen 1やfibronectinはLATマウス硬膜で発現亢進していた。TGF-β/SMADシグナルが肥厚硬膜で亢進しており、抗TGF-β1作用を持つイルベサルタンを3週間経口投与したところ、硬膜炎と肥厚が完全に抑制された。硬膜炎症に対する治療効果はガドリニウム造影MRIを用いた放射線学的解析でも裏付けられた。ヒト剖検組織を用いた病理学的検討では、IgG4関連HP患者のみならず、ANCA関連HP患者や特発性HP患者の硬膜でもTGF-βやSMAD2/SMAD3が発現亢進していることを見出した。以上より、LATマウスはHPのなかでもIgG4関連HPの動物モデルとなり得ることを世界で初めて証明し、TGF-βシグナルを標的とした新規治療がヒトHPに応用できる可能性を初めて実験的に提示した。
