脊髄損傷患者の血圧調節失調を克服するためのバイオニック血圧制御システムの開発

Research Abstract

【目的】申請者は血圧情報に基づき交感神経を電気刺激することで起立性低血圧を防ぐバイオニック血圧制御システムの開発に成功している。本研究はその基盤技術を利用し脊髄損傷患者(主として頸髄損傷患者)の重篤な起立性低血圧を克服する、バイオニック血圧制御システムの基盤技術の開発を目的とする。【進捗概要】動物実験で成功しているバイオニック血圧制御システムは後腹膜の腹腔神経神叢あるいは脊髄を直接電気刺激することで血圧を制御した。しかしながら、臨床応用のためには体表の刺激で血圧制御できることが望ましい。そのため、初年次には経皮的な電気刺激による血圧制御法を開発するとともに、そのために特化した経皮的電気刺激システムの設計を行った。2年次はこの経皮的電気刺激システムを試作し、電流波形、電流幅、周波数、簡潔刺激条件、刺激部位等の臨床的に重要な刺激条件について、九州大学医学研究院倫理審査委員会の承認をうけて実際の患者で、その最適化を試みた。その結果、適切な刺激条件や部位を用いることで経費的なバイオニック血圧制御システムで、起立性低血圧を10分程度は完全に防ぐことが可能になった。しかしながら、経皮的な電気刺激が血圧を増加させる機序が必ずも明快ではなく、さらなる最適化の可能性を残している。そこで3年次以降は一層の長時間の安定した血圧制御を行うための刺激条件の最適化、および負帰還をかけるための血圧測定法の開発を行い、最終年度の実用化を目指す。
