Over the past five years, I have collected patient narratives from 35 foreign residents who visited a Japanese hospital and encountered various difficulties. A preliminary thematic analysis of these narratives revealed differing expectations regarding informed consent. Many patients reported receiving care that was neither explained nor consented to, highlighting a significant gap in patient-provider communication. This finding became a central theme in the fourth-year elective course I designed and teach.
Although only about 10% of students enroll in this elective, the majority of participants find the narratives and topics instrumental in raising awareness about the importance of advocating for patients whose first language is not Japanese. The course promotes understanding of diverse expectations and the necessity for informed consent in medical care.
This year, I presented the course, its themes, and the free website containing the narratives and transcripts at five domestic and two international conferences. These presentations aimed to disseminate the findings and promote best practices in patient care for non-native speakers.
This year, I plan to complete the thematic analysis and publish the results. I also plan to find additional funding to collect additional narratives and conduct deeper interviews with foreign-residents who've experienced severe illness while in Japan.
