Outline of Annual Research Achievements

基礎研究では、Dahl salt-sensitive rat（DSラット）を用いた検討を行った。すなわち、DSラットを１）通常食群、２）高食塩食群、３）高食塩食＋テンポール（3 mmol/L in drinking water）投与群、４）高食塩食＋エプレレノン（100 mg/kg/day）投与群に分け、4週後に腎臓を採取した。高食塩負荷により組織学的な尿細管障害とともに酸化ストレスの増大がDSラットで認められたが、テンポールおよびエプレレノンにより有意に抑制された。腎MDA および4週後の尿中vanin-1はテンポールおよびエプレレノンにより有意に低下するとともに、4HNEおよび近位尿細管に局在するvanin-1の発現が減弱した。vanin-1は高食塩により酸化ストレスに起因する腎障害に関与し、その障害は近位尿細管特異的であることが示唆された。
臨床研究では高血圧患者を対象として尿中vanin-1を定量し、腎機能指標との関連を調べた。尿中vanin-1はeGFR < 60 mL/min per 1.73 m2を呈する患者の方が、eGFR ≧60 mL/min per 1.73 m2よりも有意に高値を示した。尿検体中のナトリウム濃度からTanaka式を用いて塩分摂取量を推算したが、eGFR < 60 mL/min per 1.73 m2を呈する患者とeGFR ≧60 mL/min per 1.73 m2を呈する患者との間に有意差は認められなかった。また、両群における血圧値に有意差は認められなかったが、eGFR < 60 mL/min per 1.73 m2を呈する患者では尿中vanin-1と収縮期血圧値との間に有意な相関が認められた。本臨床研究では食塩感受性高血圧者の同定には至らなかったが、尿中vanin-1は高血圧患者におけるeGFRと関連することが明らかになった。
