Outline of Annual Research Achievements

本研究の目的は、がんの最適医療のために必要な病理学的指標の確立である。肺癌は、個別化医療が進み、活性化キナーゼ阻害剤や免疫チェックポイント阻害剤（ICI）の開発は、一部の患者に大きな恩恵をもたらしている。治療方針の最適化のためには、がんの個性を読み解き、適した治療を選択する指標（バイオマーカー）を確立する必要がある。ICIでPD-L1が普及しているが、その性能は不十分で、より精度の高いバイオマーカーの探索と確立は喫緊の課題である。
本研究では、ICIに対する病理学的予測指標を探索し、その意義を検討した。（１）ICI治療を受けた肺がんの病理検体の特徴を検討し、病理形態学的な指標（核異型度、炎症の程度）が予後予測指標となることを見出した。病理医の目視評価を客観化するために、HE染色標本のデジタル画像を作成し、がん細胞の形態学的特徴量（MBM)を画像解析により定量化した。前年までに、MBMは決定木法モデルで妥当な性能が期待できることを示したが、本年度は、症例数を増やした深層学習（DNN）による最適化と検証実験を実施した。DNN-MBMは従来のバイオマーカーであるPD-L1を超える治療効果および予後予測性能を示した。（２）生物学的意義の検討のために、がんのMBMと腫瘍浸潤免疫細胞（TIL）、ゲノム情報、臨床情報との関連性を統計学的に解析した。MBMは、遺伝子変異、TMBと中等度に関連、PD-L1 IHCと弱く関連した。MBMは全例で解析可能であり、TMB, PD-L1と比して解析成功率が高かった。（３）非小細胞癌の免疫染色によるTIL検討の結果、CD8陽性Tリンパ球の腫瘍内への浸潤は予後、ICIの治療効果と関連することを示した。
以上より、腫瘍組織の病理学的特徴は、客観的に抽出可能な因子であり、PD-L1とは独立しており、実臨床に応用可能な新規バイオマーカーとして有望といえる。

新型コロナの流行により、ゲノム解析等の遅れがあり、統合解析にやや遅れが生じた。

Strategy for Future Research Activity

本研究で、形態学的バイオマーカーはICI治療効果予測に有用であることが示された。これまでの研究対象は、ICI単剤による解析であったが、実臨床ではICIと化学療法の併用療法が登場し、治療法の変化に対応できるかどうかを検索する必要がある。また、実用化を目指し、その他のバイオマーカーとの役割検証、検体種、検体採取時期などの解析結果への影響、AI画像解析の自動化など、課題について研究を進める必要がある。平行して、殺細胞性抗癌剤やキナーゼ阻害剤など他の治療法に対する本技術の応用可能性も検討を進めたい。

新型コロナの流行により、研究解析の遅れが生じたため。次年度には、解析、研究を完了すべく鋭意研究を進めている。
