Outline of Annual Research Achievements

今までのCTに比べてはるかに高い空間分解能(解像度)を有する超高精細CTに逐次近似再構成法を併用することで、非常に微細な構造や病変を高画質で描出できるので、さまざまなCT検査で大幅な診断能の向上とX線被ばくの低減が期待されている。本研究では、脳、頭頚部、体幹部、骨関節などの領域で超高精細CTと逐次近似再構成法の併用がもたらす画質の改善と被ばくの低減を検討し、その臨床的有用性を解明する。
令和5年度は引き続き頭部CT血管造影では脳動脈穿通枝、側頭骨CTでは中耳・内耳の微細構造、胸部CTでは仮想気管支鏡などを主な対象として、上記の臨床的有用性につき招待講演などで発表している。
主に体幹部CTにおけるさらなる画質向上や被ばく低減の目的に人工知能による深層学習を応用した画像再構成法も導入されている。この深層学習再構成法は高性能な逐次近似再構成法により再構成された画像データを教師画像としている。本再構成法も画像ノイズを著明に低減できるとともに、逐次近似再構成と比較して計算時間が短く、自然な画質が維持できるという特長がある。造影腹部骨盤CTでは逐次近似再構成法に加えてこの深層学習再構成法の併用による大幅な被ばく低減、造影剤減量の可能性をファントム実験、臨床パイロット研究による検討を行い論文化した。具体的には、超高精細CTと深層学習再構成法は通常に比し1/3程度の被ばく、40%減の造影剤量でも高解像度でありながら十分な画質を担保できることが示された。この成果についても招待講演などで発表している。
さらに、今までのCTにおいても主に空間分解能を改善すべく新たな深層学習再構成法が導入されている。これは超高精細CTで撮影し、従来の深層学習再構成法により再構成された画像データを教師画像としている。この冠動脈CTにおける臨床的有用性も国際学会で発表している。
