| Outline of Annual Research Achievements |
1. 本研究期間に、腹膜透析(PD)患者の腹膜傷害への治療介入を期待して、現在、抗補体薬としてヒトに既に使用されているC1インヒビターを用いて、真菌性腹膜炎の実験動物モデル（ラット）における腹膜傷害の抑制効果明らかにした。その成果を学会発表、および国際的科学雑誌に投稿し現在in-pressの状況である。さらに、現在、腹膜傷害から被嚢性腹膜硬化症（EPS）に至る過程への補体終末経路の関与を、動物モデルを用いて進めるために海外共同研究者から補体関連蛋白の遺伝子改変動物、中和抗体等を用いて検討を進めている。一方で、腎不全動物モデルを作成し、腹膜傷害進展への尿毒症の効果の検討も現在進めている。
2. PD排液を用いて、腹膜が実際に受けている影響を明らかにするために、これまでに行っていた補体活性化産物の一つsC5b-9の測定に加えて、補体活性化路の基本路であるAlterative pathwayに注目して、ELISA系を用いて腹膜透析液排液中のC3a、プロパジンの測定を開始し、現在もデータの蓄積を進めている。
3. 現在、PDが、生体の腹膜に与える影響、特に補体活性系とその制御系のバランスに与える影響の検討を進めている。本年度はPD透析導後1～3年の間のPD患者の排液中の腹膜中皮初代培養細胞（HMPC）を用いて、膜補体制御因子（CRegs）の発現の変化について検討し、CRegsの中でCD46やCD59と異なり、CD55が時間経過と共に中皮細胞への発現が増強することを確認した。本研究期間に結果の一部を国際学会で発表した。さらに長期の5年以降のPD患者の排液、PD導入期の排液、腹膜炎発症による影響を明らかにするために、現在も検体の収集と解析を進めている。また、腹膜がPDにより影響を受けると考えられる個々の要因についてMet-5A細胞を用いて検討を進めている。
