Outline of Annual Research Achievements

研究最終年度である本年度は、蘇生中止基準のさらなる精度向上を目指し、病院前因子の詳細な解析を試みた。まず、口頭指導による救命処置が初期心電図および転帰に与える効果について解析した。その結果,口頭指導による救命処置群と口頭指導の無い救命処置群の間で,ショック初期心電図割合と1か月後の転機良好割合において有意な差は無いことが判明した(Eur J Emerg Med誌掲載、欧州救急医学会発表)。これは、過去に救命処置訓練の無い救助者であっても、口頭指導による救命処置は訓練された救助者と同等の効果があることを示している。次に、救命処置の方法について解析した。国際的蘇生ガイドライン(ILCOR,2020)では、胸骨圧迫のみの救命処置を推奨しているが、このガイドライン作成の基となった3編のランダム化比較試験(RCT)は、いずれも15:2の胸骨圧迫と人工呼吸による救命処置法による検討で、その割合が30:2に変更になってからのRCT研究は無い。そこで、成人院外心停止例を対象に、口頭指導による標準的救命処置と胸骨圧迫のみの救命処置の蘇生後転帰を比較した。その結果、口頭指導は標準的な1次救命処置の方が胸骨圧迫のみの方法より、神経学的転帰良好と有意に関連していることが判明した(Crit Care誌掲載、欧州集中治療医学会発表)。小児院外心停止においても検討した結果、同様の結果であった(Resuscitation誌掲載、国際救命救急学会発表)。さらに、小児において、非ショック初期心電図からの調律変換後除細動が転帰に与える影響について検討した。その結果、除細動実施時間が救急隊の救命処置開始から9分以内であれば、初期心電図が心静止か無脈性電気的活動に関わりなく、非除細動群と比較して1か月後の神経学的転帰良好割合が高いことを明らかにした(Resuscitation Plus誌掲載、欧州不整脈学会発表)。
