Outline of Final Research Achievements

The aim of the current study was to develop a Japanese version of the Values History Form using the Delphi method with care managers at long-term care facilities for the elderly. As a result, 22 items were identified: a “Comfortable living environment,” “What is important to you,” “What your doctor has told you about your health problems,” “Your trust in your doctor,” “Your family’s assent to the end-of-life care you want,” “Use of life-sustaining measures” “The characteristics of your decision-making,” “The characteristics of your communication with others,” et al.. These items comprise a unique Japanese version of the Values History Form.
The Japanese version of the Values History Form should be used before a serious health problem develops, it should be periodically readministered since values change, and it needs to be shared with relevant parties.

Academic Significance and Societal Importance of the Research Achievements

近年は超高齢化や単独世帯の増加に伴い、本人の終末期医療に対する意思を如何に把握するかは重要な課題である。自分の意思を顕在化させ関係者に示す方法として米国ではバリューズヒストリー(以下、VH)が用いられている。このVHは終末期医療の意思決定の根拠となる価値観を示すものであり、わが国の文化に即した日本版VHの開発が望まれる。   本研究により開発された日本版VHは、病名や病気の見通しなどについて本人に事実を伝えるとは限らないわが国の状況や、集団の意向を重視するといった日本人の意思決定の特徴が反映しており、わが国の終末期医療の意思決定の一助になる。
