Outline of Final Research Achievements

We investigated changes in basal metabolic rate (BMR) in pediatric patients with chronic kidney disease before and after kidney transplantation using indirect calorimetry. Both pre- and post-transplant BMRs were higher than the standard BMR values for actual age but were comparable to the standard BMR values adjusted for body size. Furthermore, no significant changes in BMR were observed before and after transplantation. Regarding the respiratory quotient, results indicated a significant increase post-transplant compared to pre-transplant values.

Academic Significance and Societal Importance of the Research Achievements

本研究により、小児慢性腎臓病患者の適切な栄養管理に関し、実際の年齢ではなく、体格に合わせた年齢の基礎代謝量に近いことが確認された。多くの低年齢小児慢性腎臓病患者では実年齢より体格に合わせた年齢の基礎代謝量のほうが高く、その分必要エネルギー量も増すということになり、腎臓病による成長障害を改善させるうえでも必要エネルギー量を把握するということが重要と考えられた。
