Elucidation of the pathophysiology of prefrontal cortex in patients with treatment-resistant depression using transcranial magnetic stimulation neurophysiology

Outline of Annual Research Achievements

まずはじめに本研究プロトコルの修正申請を行い、倫理員会から承認を得た。その後速やかに経頭蓋磁気刺激‐高精度脳波同時計測(TMS-EEG)実験システムおよびMRIガイド下ニューロナビゲーションシステムの構築を行い、さらに、TMS-EEG実験を円滑に実施できるようにスタッフの実地訓練を行ってきた。また、関連医療機関と連携しながら、被験者のリクルート体制の確立も同時並行で進めていった。

現在、健常被験者9名及びうつ病患者10名を本研究に組み入れ、MRI撮像、TMS-EEG実験、研究採血、包括的臨床評価・認知機能検査を行い、同時期に左背外側前頭前野におけるコリン作動性神経生理機能(SAI)・GABAA作動性神経生理機能(SICI)・グルタミン酸作動性神経生理機能(ICF)・神経可塑性神経生理機能(PAS)パラダイムの生理実験を実施した。

健常被験者に対するプレリミナリー解析では、コリン作動性神経生理指標(SAI)において刺激部位においてTMS誘発脳波成分(TEP)の中のN100成分の減弱を認め、我々の先行研究結果(Noda et al., 2016)を再現する結果が示された。TMS後100ms時点でのトポグラフィでも、TMS誘発脳波結果と同様に、SAIパラダイムではテスト刺激条件と比べ、左背側前頭前野の電位低下が小さいことが示された。さらに、GABA(A)受容体作動性神経生理指標(SICI)およびグルタミン酸NMDA受容体作動性神経生理指標(ICF)に関するプレリミナリー解析では、SICIではP60成分の減弱を認め、ICFでは早期成分の増強が起きている可能性が示された。

Reason

目標人数のうつ病群30名・健常群30名のうち、各群ともに3分の1までリクルート及び実験が終了しているため、本若手研究の3ヵ年計画としては、概ね順当なペースで進行しているものと考えられる。

Strategy for Future Research Activity

今後はうつ病患者に対するTEP解析を行い、群間比較や各症状との相関解析等を行っていく予定である。さらに、目標人数のデータが集まり次第、マルチモーダルMRI画像解析や末梢血のメタボロミクス解析等も進めていく予定である。
