Outline of Annual Research Achievements

2019年度は前年度以上にTMS-EEG実験の遂行が円滑となり、リクルートも順調に進んだ。2019年3月時点で、治療抵抗性うつ病患者の組み入れは、既に目標人数の30名に到達し、対照健常被験者は16名まで組み入れと実験を終えている。

本研究では、TMS-EEG実験と同時期にマルチモーダルMRI撮像、研究採血、包括的臨床評価・認知機能検査も実施している。TMS-EEG実験では、左背外側前頭前野におけるコリン作動性神経生理機能(SAI)・GABAA作動性神経生理機能(SICI)・グルタミン酸作動性神経生理機能(ICF)・神経可塑性神経生理機能(PAS)パラダイムを実施しているが、これらの生物指標・臨床認知尺度も網羅的・包括的に測定している。

現時点でのプレリミナリー解析結果では、治療抵抗性うつ病患者では、健常群と比べ、ICF指標におけるグルタミン酸作動性興奮機能が相対的に上昇している可能性が示唆された。また、PAS指標による神経可塑性機能に関しては、治療抵抗性うつ病患者では、健常群と比べ、神経可塑性の誘導発現が低下している可能性が示唆された。この結果は、我々の先行研究結果(Noda et al., Depress Anxiety. 2018)を再現するものとなった。

Reason

うつ病群に関しては既に目標人数の30名の組み入れと実験を終えており、健常群に関しても目標人数の半分以上の計測を終えている。プレリミナリー解析も順次行っており、非常に順当なペースで研究が進行しているものと考えられる。

Strategy for Future Research Activity

今後は健常者のリクルートとTMS-EEG実験を進めTEP解析を行い、うつ病群と健常群の群間比較や各症状との相関解析を行っていく予定である。さらに、マルチモーダルMRI画像解析や末梢血メタボロミクス解析も順次進めていく。

Remarks

MTRラボでは、精緻な臨床評価、磁気共鳴画像(MRI)を用いた多様な神経画像モダリティと経頭蓋磁気刺激法(TMS)と高解像度脳波計(EEG)を組み合わせたTMS-EEGによる神経生理モダリティ、さらにはデータサイエンスの手法を用いて、精神疾患のバイオマーカーの同定、新たな診断補助技術の開発、治療効果発現機序と治療転帰予測、早期発見と予防、より効果的な新規治療法といったテーマに取り組んでいます。
