Outline of Annual Research Achievements

本研究では、治療抵抗性うつ病患者を対象にTMS-EEG神経生理実験を中心として、包括的臨床評価・認知機能検査、マルチモーダルMRI撮像、メタボロミクス用研究採血を網羅的に実施しており、2020年度中に目標人数であったうつ病患者30名および健常コントロール30名の組み入れ、実験、計測を完了させた。
本研究課題のメインであるTMS-EEG実験では、左背外側前頭前野からGABA(A)受容体介在性神経生理機能(SICI)・グルタミン酸受容体介在性神経生理機能(ICF)・神経可塑性神経生理機能(PAS)パラダイム等を実施したが、本研究の成果としては以下のものが得られた。1)治療抵抗性うつ病患者では、健常群と比べ、ICF指標におけるグルタミン酸作動性興奮機能が相対的に上昇している可能性が示唆された。2)PAS指標による神経可塑性機能に関しては、治療抵抗性うつ病患者では、健常群と比べ、神経可塑性の誘導発現が低下している可能性が示唆された。この結果は、研究代表者が以前行った研究結果(Noda et al., Depress Anxiety. 2018)を再現する形となった。
うつ病では、前頭前野の神経可塑性が低下していることが知られているが、神経可塑性とその後のTMS療法との関係性について、我々が予備的に解析した結果からは、ベースライン時点における左背外側前頭前野のPAS指標の神経可塑性が相対的に低下している患者の方がTMS療法に対する治療反応性が高い可能性が示された。
今後の研究の方向性としては、うつ病の前頭前野におけるPAS神経可塑性指標をはじめとしたTMS-EEG神経生理指標を網羅的に解析することによって、うつ病の病態生理を精緻に解明していくことを目指している。
