Outline of Final Research Achievements

This study aimed to analyse the functional lymphovascular network and tissue drainage in the anorectal region. We performed microanatomical evaluations wit India inc injection using flesh cadaver, and performed intraoperative indocyanine green fluorescence imaging analysis for patients with rectal cancer. Results of these analyses suggested a widespread functional lymphatic flow between rectal muscle fibres and the surface of levator ani muscle.

Academic Significance and Societal Importance of the Research Achievements

進行下部直腸癌治療において、局所再発率の改善が課題であり、集学的治療など様々な治療が試みられている。下部直腸肛門管領域のリンパ流に関しては不明な点が多く、本研究では骨盤底、特に肛門挙筋表面に広範囲に広がるリンパ流が示唆された。本研究により、解剖学的観点から治療方針を考察することが可能になると考えられる。手術治療のみではこの広範なリンパ流による腫瘍進展を制御することは困難であり、術前療法を中心とした集学的治療が妥当である可能性が高い。
