Identification of genotypes in immune-related genes predicting outcomes in patients following allogeneic hematopoietic cell transplantation

Outline of Annual Research Achievements

2009年6月から2018年12月の間に当施設で移植後シクロホスファミドを用いた血縁HLA半合致同種造血細胞移植（PT/Cyハプロ移植）を受けた患者を対象とし、患者とドナー各々、計16のKIR遺伝子型のタイピングを行い、計91例でKIR遺伝子型を同定できた。34例（37%）が寛解期移植であった。移植前寛解群では、KIR2DS1陽性ドナーからの移植はKIR2DS1陰性ドナーからの移植と比較して再発率及び生存率が統計学的に有意に良好であった（2年再発率 9.2% vs 42%、P = 0.037；2年生存率 83% vs 34%、P = 0.010）。一方、移植前非寛解群では、KIR2DS1陽性ドナーからの移植は再発率及び生存率を改善させなかった。以上の結果より、PT/Cyハプロ移植において、移植前寛解群ではKIR2DS1陽性ドナーによる再発率及び生存率の有意な改善を認めたが、非寛解群では認めなかった。KIR2DS1を介したドナーNK細胞刺激によるGVL効果は、低腫瘍量の症例で発揮されると考えられた。本研究結果は、PT/Cyハプロ移植における最適なドナー選択アルゴリズムの確立に寄与する可能性がある（Biol Blood Marrow Transplant. 2019 Dec 31. pii: S1083-8791(19)31733-1.）。
