| Research Abstract |
(1) 視覚的にPIB-PET陽性の18名と陰性の9名を対象に、皮質のMCDVR (Mean Cortical distribution volume ratio)を求めた。視覚的に陽性群のMCDVRは1.57、陰性群は0.98で、カットオフ値は1.3となった。(2) 初回PIB-PET検査から2週間以内に同一条件で再測定を行い、MCDVRの測定変動を求めた。平均誤差7.6%で、PIB-PETの再現性試験では測定誤差が10%以内であった。(3) PIB-PETで陽性率は、AD群では86.7%、MCI群では51.7%、健常者では33.3%で、AD群にもPIB-PET陰性者が存在し、逆に健常者と思われている中にも陽性者のいることが明らかになった。(4) Aβ42は、AD群およびMCI群で、PIB-PET陽性群が有意(p<0.01)に低値を示した。Aβ42/40比も、AD群およびMCI群で、PIB-PET陽性群が有意(p<0.01)に低値を示した。逆に、T-tauはAD群(およびMCI群において統計学的に有意な高値(p<0.03)を示した。p-tauも同じ傾向があり、MCI群においてPIB(+)群は高値を示した(p<0.01)。また、Aβ42(r=0.524)およびAβ42/40比r=0465)とMCDVRとの間には、有意の負の相関関係を認めた。(5) PIB-PETを平均16.9か月後に再検査を実施した。MCDVRは、AD群では1.35から1.35、MCI群では、1.32から1.37で、有意な変化は認めなかった。
<まとめ>PIB-PET検査は、認知症髄液マーカーと相関関係があり、ADの診断に役立つ可能性がある。しかし、PIB-PET陰性のAD症例がある。MCDVRの変動は10%以内であるが、1年半程度の経過では変動を把握できない可能性のあることが明らかになった
