| Research Abstract |
(1)OCT画像における組織のカラー表示化
剖検例の冠動脈のOCT画像と組織像とを比較し、OCTシグナルの減衰・分散などの解析を組み合わせることにより、冠動脈プラークを線維性・脂質性・石灰化などに分離することが可能で、そのデータに基づいて冠動脈プラークをカラー表示し識別することが可能であることを確認した。特に、血栓に関しては、白色血栓と赤色血栓の判別ができるだけでなく、減衰係数から血栓の赤血球濃度まで判定できることを証明し、日本循環器学会にて報告した。動脈硬化家兎(WHHL)においても組織診断とOCT画像を比較し、動脈硬化病変評価の精度を検討し、両者がよく一致することを確認した。また、内膜下のマクロファージ集積や泡沫細胞の同定の可能性も確認した。
(2)臨床例における不安定プラークの評価
臨床例において、急性冠症候群症例の責任血管と非責任血管のOCT像を記録し、従来のグレイスケールOCTを用いて責任冠動脈のプラークを解析すると同時に非責任血管において不安定プラークを判定した。従来の診断法に比較してOCTではプラークの破裂・潰瘍形成・血栓の存在診断に優れており、不安定プラークの特徴とされる線維性被膜の薄いプラーク(TCFA)の診断に優れていることを確認し、米国心臓病学会誌(J.Am. Coll. Cardiol.)米国心臓協会誌(Circulation)に報告した。その他、小さな散在性の石灰化の分布なども検討し米国心臓病学会などに報告した。
