純粋レヴィー小体病の認知障害に関する、動的神経病理学的研究

Research Abstract

1)後方視検討として、純粋レヴィー小体病の臨床病理学的検討を行った。純粋レヴィー小体(LB)病とはLBD=LB型認知症(DLB)および、認知症を伴うParkinson病(PDD)で、病理学的に他のアルツハイマー病変化や脳血管障害などを伴わない群である。対象は高齢者連続剖検1640例(男性827、女性694例、平均80.7歳)。全例に免疫組織学的な検討を加え、LBを認知症の唯一の病理基盤とする症例を抽出した。その結果、LBD78例中、純粋型は19例(男性12、女性7例、平均85.0歳)と少なかった。16例にパーキンソニズムを認め、うち8例はPDDであった。著しい幻覚は6例に認め、明瞭な起立性低血圧は2例にあった。DLB 11例中2例でL-Dopaを使用され有効であった。死亡時平均CDRは2.3、最終ADLは歩行可能1例、杖歩行2例、車椅子使用4例、床上生活12例。平均罹病期間は8.9年であった。ApoE e4アリル頻度は検索群全体で0.1、純粋LBDで0.06。LBスコアでは脳幹型0例、移行型16例、新皮質型4例であった。全例に末梢自律神経系LB病変を認めた。よってAlzheimer病変の影響のないLBDの臨床病理像を明らかにし、LB単独の認知症関与様式を明らかにした。
2)前方視的検討として、パーキンソン病関連疾患・認知症群のパス入院をすすめ、髄液バイオマーカー、PIBPETの解析により、純粋型レヴィー小体病の症例を抽出し、後方視的検討結果と比較することを開始した。1)と2)の統合については次年度に検討を行う。
