Research Abstract

BMI1 は、hedgehog(Hh) pathway のDownstreamに位置し、癌幹細胞の増殖、血液幹細胞、神経細胞、腸管細胞のself-renewalに重要な役割を果たすことが知られている。近年の私どもの報告では、Hh signal活性化は、進行食道癌の早期再発が予測されることを報告してきた。しかしながら術前化学放射線療法(以下CRTと略する)を行った食道扁平上皮がんにおけるBMI1発現の臨床的意義については明らかにされていない。食道扁平上皮がんと診断され、術前CRTを行った78名の患者を検討した。その結果、24名の患者はBMI1 陽性であった。BMI1 陽性患者の予後は、そうでない患者に比べ有意に不良であった(mean DFS time : 16.8 vs 71.2 months ; 3-yr DFS : 13.3% vs49.9%,p=0.002 ; OS time 21.8 vs 76.6 months ; 3-yr OS 16.2% vs 51.7%,p=0.0005)。p16 とBMI1 発現の相関性は認めなかった。
今回の研究は、(1)BMI1発現は、進行食道癌CRT後切除患者の早期再発と予後のPredictorとなりえる。
(2)Hh pathwayにおけるBMI1 signalの活性化は、進行食道癌CRT後の癌再発と進行のpromotionする可能性がある。
