脂肪細胞由来因子の心血管リモデリング制御機構の解明と治療への応用

Research Abstract

1. アディポネクチンの心筋梗塞後の心リモデリングにおける役割検討:野生型(WT)マウス、アディポネクチン遺伝子欠損(APN-KO)マウスに心筋梗塞モデルを作成。APN-KOマウスでは、術後の左室腔が著明に拡大、心機能低下、虚血領域における血管新生の低下を認めた。アデノウイルスを用いて、アディポネクチン濃度を上昇させると心筋梗塞モデル作製後の心機能が改善。(J Mol Cell Cardiol, 2007)。
2. 血管内皮前駆細胞(EPC)の動員や機能に対するアディポネクチンの役割検討:APN-KOマウスとWTマウスに下肢虚血モデルを作成。APN-KOマウスでは、WTマウスに比較し、血流回復能が悪く、モデル作成後EPCの骨髄からの動員が低下していた。アディポネクチン濃度を増加させたところ、EPCの骨髄からの動員は増加。ヒト末梢血を利用した培養実験系ではアディポネクチン蛋白がEPCコロニー形成を促進、管腔形成能や遊走能を増強させた(FEBS Lett 2008)。また、カロリー制限により、アディポネクチン産生促進がおこり、同様に血管新生促進作用がおこることを明らかにした(J Biol Chem. 2009)。
3. ヒト心筋梗塞患者におけるアディポネクチン濃度、EPC動員と心リモデリングとの関連検討:心筋梗塞患者では、アディポネクチン濃度とEPCのマーカーである、CD34陽性細胞の数とが、有為な相関関係を持つことが示され、アディポネクチン濃度が高値な患者で、心筋梗塞発症6ヵ月後の梗塞サイズが小さく、心機能が良いことが明らかになった(Am J Cardiol, 2008)。
以上1-3の結果から、アディポネクチンは心血管リモデリングに予防的に働き、その機序として、血管新生促進作用が関与している事が考えられた。
