| Research Abstract |
造血幹細胞移植(HSCT)後の二次性固形腫瘍に関して、成人血縁者間移植患者を対象に頻度およびリスク因子の検討を行った。
2007 年までに初回血縁者間HSCT が実施され、日本造血細胞移植学会全国調査に報告された16 歳以上の成人9388 例(移植年1974-2007 年)を対象とした。
80 例において二次性固形腫瘍が確認され、移植後1 年以内の発症が12 例に認められた。累積発症率は5 年において0.6%(95%信頼区間[CI], 0.5-0.8%)、10 年において1.2%(95%CI,0.9-1.5%)であった。
5 例において2 種類の固形腫瘍の発症が見られた。日本人の一般人口データとの比較では、発症リスクは一般人口と比べ有意に高く(標準頻度比(SIR) = 1.3, 95%CI,1.1-1.6)、特に口腔癌(SIR = 12.5, 95%CI, 7.7-19.1)、食道癌(SIR = 6.4, 95%CI, 3.3-11.2)におけるリスクが高かった。
移植後1 年以上生存した患者5776 例での二次性固形腫瘍発症リスク因子の検討では、TBI、慢性GVHD 含め有意なリスク因子はなかった。
癌腫によっては一般人口に比べて高頻度の発症率であることを示したことは大きな意義がある。移植後患者のフォローにおいて二次癌の可能性を念頭に置いた対応が必要であること、さらに定期的なスクリーニングの必要性を示した。
