Comprehensive validation of circulating microRNA signatures for Moyamoya disease early detection as an epigenome blood biomarker

Outline of Annual Research Achievements

もやもや病の多様な臨床像は、感受性遺伝子RNF213多型だけでは説明しきれない。研究代表者は末梢循環エピゲノム制御機構microRNA(miRNA)が様々な疾患の分子病態に関与することに着目し、miRNAによるエピゲノム環境が、もやもや病の発症素因を修飾して多様な病態臨床像に関わるとの仮定した。全160検体(もやもや病確定診断例82名と、年齢性別をマッチさせた健常対照者78名)のmicroRNA発現を、カスタム定量PCRパネルで測定した結果、関心17種miRNAのうち、 8種のmiRNAを全対象者で定量しえた。発現量を主成分分析した結果、もやもや病患者群と健常対照者群の2集団で末梢血miRNA発現profileが異なることが確認された。2群間での発現変動解析の結果、hsa-miR-328-3pは、先行の双生児研究で得た成果と同様、もやもや病群で有意な発現亢進(発現変動比 1.9倍、p<0.0001)を認めた。臨床指標(発症年齢・性別・家族性・RNF213創始者変異・発症病型・鈴木病期・Periventricular anastomosis・PCA involvement)との関連を解析した結果、発症年齢・家族性・RNF213変異・鈴木病期・PCA involvement とmiR328発現量有意な関連を認めた。hsa-miR-328-3pのくも膜における発現をFISH法により測定した結果、比較対照群とした非もやもや病(主に未破裂脳動脈瘤患者)くも膜ではほとんど認められないmiR-328-3p発現が、もやもや病患者くも膜では認められた。これらの研究成果から、末梢血を安定して循環するmicroRNAは、もやもや病のエピゲノム血液バイオマーカーとなり、特にhsa-miR328-3pは、もやもや病くも膜上でその発症素因を修飾して、血管病期の進行に関与している可能性が示唆された。
