Outline of Final Research Achievements |
In order to understand the perinatal medical care delivery system in each prefecture, we surveyed the current status of delivery facilities in each area, measured the travel distance from the representative point of users to delivery facilities, and conducted a feasibility study on consolidation. The survey covered three prefectures in Kansai (Shiga, Kyoto, Hyogo), three prefectures in Kansai (Osaka, Nara, Wakayama), five prefectures in Chugoku (Tottori, Shimane, Okayama, Hiroshima, Yamaguchi), four prefectures in Shikoku (Tokushima, Kagawa, Ehime, Kochi), and four prefectures in Kyushu (Fukuoka, Saga, Nagasaki, Oita). Travel distances tended to be shorter in secondary medical care areas with a high number of births and longer in secondary medical care areas with a low number of births. Some centers (comprehensive and regional perinatal maternal and child care centers) did not reach the “desirable level” of the number of physicians.

Academic Significance and Societal Importance of the Research Achievements |
日本では出生数の減少が今後も継続すると予想されている。医療機関における医療の高度化は、必要な医療従事者数を増加させ、2024年度より開始する働き方改革は、医療従事者が提供できる労働時間に上限を設ける。 出生数の減少、医療の高度化、働き方改革等は、分娩施設の集約化を促すだろう。その結果、医療機関間、あるいは、患者の居住地から高次医療機関への移動距離の延長をもたらす可能性がある。一方、施設あたりの医師数が増加し、その施設の安全性が高まって医療の質が向上するかもしれない。高次医療機関の医療の質や安全性を高めることを目的とする集約化を考えるとき、移動距離を把握する必要がある。
