Outline of Annual Research Achievements

心臓移植後患者30人に対して, 心筋障害の有無、心機能評価、冠動脈狭窄の有無を確認するため心臓MRIを施行した。心筋障害は移植後拒絶反応や冠動脈狭窄・微小循環障害に関連する心筋浮腫、炎症、虚血を対象とした。そのために、造影剤を使わない非侵襲的な心筋T1, T2マッピングとIVIM技術を使って心筋障害の程度を定量的に解析した。患者全員から心筋生検を実施し、得られた組織学的な拒絶反応の程度、リンパ細胞浸潤に関連するQuilty病変と心臓MRIの定量的指標を比較した。その結果、病理学的にQuilty病変の証明された患者で、心筋T1値の有意な延長が見られることがわかった。Quilty病変は、拒絶反応発症の契機や移植後使用する免疫抑制剤による心筋障害との関連が報告されており、心筋T1マッピングがこれらを検出するための非侵襲的で患者負担の少ない手段となることが期待できる。一方、T2マッピングとIVIM技術と病理学的所見との関連は得られなかった。本研究対象者には、重症の拒絶反応、心機能低下や冠動脈狭窄を持つ患者はおらず、軽微な心筋障害の有無が結果に影響した可能性はある。
