| Outline of Annual Research Achievements |
膵癌の予後は極めて不良であり予後改善に向けた治療戦略が必須である。近年、膵癌腫瘍内及び腫瘍周囲の免疫が予後と相関がある事が報告されている。
当院の膵癌の手術検体を用いて、膵癌の腫瘍内のCD4陽性T細胞、CD8陽性T細胞を染色し予後との関連性を検討した。腫瘍内部のCD4、CD8陽性T細胞が多い群で明らかに予後良好である事を見出した。それに加えて、膵癌の内部や辺縁に存在するTLSというリンパ球の集簇の有無にも着目した。TLSのある膵癌では有意に予後が良好であった。さらに、TLSの存在する膵癌では、特に5-FU系の抗がん剤であるTS-1の薬剤感受性が高いことも見出した。
現在、手術例の便検体の収集が順調に進んでおり、腫瘍内の免疫担当細胞やTLSと腸内細菌との関連性を検討していく予定である。
