実験的高血圧マウスモデルによる破骨細胞形成および矯正学的歯の移動の解析

Outline of Annual Research Achievements

近年、高齢化に伴い、歯科矯正領域においても高血圧を有する患者が増加してきている。この高血圧は骨粗鬆症性骨折のリスクとなると報告された。また、高血圧によって起こるレニン-アンジオテンシン系の亢進は、骨吸収を増加させることがわかった。そのため高血圧治療薬であるアンジオテンシンII受容体拮抗薬(ARB)やアンジオテンシン変換酵素(ACE)阻害薬は、骨吸収を抑制することが報告された。矯正学的歯の移動では、破骨細胞が骨を吸収し歯が移動していく。これらのことから高血圧患者において、矯正学的歯の移動に影響が出ることが考えられる。本研究の目的は、高血圧および高血圧抑制剤の破骨細胞形成への影響およびメカニズムの解明、さらに矯正学的歯の移動への影響をin vitroおよびin vivoで解明することが目的である。現在までにマウスにLNAMEを飲用水にて2週間飲用させる。その後、2週間は、通常の飲用水に変える。その後、3週間は高塩分食を与え、高血圧マウスを作成することに成功した。高血圧マウスおよび正常マウスの頭蓋骨にLPSを注入し、頭蓋骨の組織切片より破骨細胞形成および骨吸収を評価した。正常マウスと比較して高血圧マウスは、破骨細胞形成および骨吸収が増加した。また、高血圧マウスの頭蓋部および血清中のTNF-αが増加した。さらに、AT1Rの発現が増加した。TNF-αが骨芽細胞に作用するとATGR1の発現が増加をin vitroで見出した。さらに高血圧マウスの大腿骨をμCTによる解析では、骨量が減少し骨粗鬆症様の所見が得られた。TNF-αの骨芽細胞への作用でAGTR1が増加するのは、p38のリン酸化を介して起こることを見出した。これらのことから高血圧になるとTNF-αが増加し骨芽細胞でのAGTR1発現を増加することを介して、RANKLの発現が増加することで破骨細胞形成が増加することがわかった。
