Outline of Final Research Achievements

This study reverled 1) ways of state government finance in post-Obamacare(Patient Protection and Affordable Care Act of 2010) Medicaid reform, 2) dynamic process of state-federal government relationship over Texas Medicaid 1115 waiver, and 3) achievements and challenges of state Medicaid reform in post-Obamacare era.

Academic Significance and Societal Importance of the Research Achievements

本研究の学術的意義は、アメリカにおいて2010年に成立した患者保護アフォーダブルケア法（通称「オバマケア」）の柱となるメディケイドの対象拡大のプロセスを、州の事例研究を軸に連邦政策の展開に沿って検討することを通して、分権的な制度改革の背景となる州財政の論理および州・連邦政府間関係のダイナミズムを解明した点にある。州財政および州・連邦政府間関係に着目した本研究の成果は、アメリカにおける医療扶助政策の方向性を明らかにするための基盤となることに加え、日本において貧困者の医療ニーズに対応した医療扶助システムを構築するための政策のあり方を検討するための有力なモデルを提示するはずである。
