Outline of Annual Research Achievements |
First, a taxonomy system for electronic medical record (EMR)-related incident analysis was developed. Particularly, this taxonomy system paid more attention on interaction failures between healthcare staff and EMR systems. Second, the developed taxonomy system for EMR-related incident analysis were applied to the collected incidents. Results showed that the frequent failures were for instance because of healthcare staff’s wrong input, missing input and lacking of data check. Latent factors behind these failures included insufficient training, interruptions, varied culture between different units/departments, etc. Third, based on all the findings, an EMR-related incident management system was developed. It includes contents related to incident reporting, i.e., what kind of incidents should be reported, to what extent; how to make analysis; corresponding to the analysis results, what kind of measures could be taken; and how to make evaluations after taking measures. Fourth, the developed EMR-related incident management system was implemented after adaptation. Effectiveness of the system was verified using indicators such as change of EMR-related incident reporting rate, appropriateness of the proposed measures, and healthcare staff and risk managers’ satisfaction. In addition to the quantitative and qualitative effectiveness measurement, usability of the system was also investigated to patient safety managers and healthcare providers as the users. Based on the results and suggestions, improvement of the system’s usability and utility was conducted.
