Outline of Annual Research Achievements

Fontan手術時に静脈圧軽減目的に作成されたfenestration（開窓）は術後遠隔期にfenestrationによるチアノーゼが問題となるため閉鎖を試みるが、試験閉鎖で静脈圧上昇や心拍出量低下がみられる場合は閉鎖することはできない。fenestrationが血行動態に与える影響は患者ごとの心機能、静脈圧、体肺血管抵抗などに大きく依存するため、fenestrationの作成・閉鎖の適応を一律に定めることは不可能である。本研究では最終的にはfenestration作成・閉鎖後の血行動態を予測し、治療適応の判断、治療後の安全なマネージメントを実現することを目的とする。
fenestrationに先立ち、循環平衡の概念に心房間交通を組み込んだ数理予測が成立することをイヌ心房中隔欠損モデルで立証できるか検証中である。
