Research Abstract

循環器疾患の発症率の推移と危険因子の寄与の大きさの推移を明らかにする目的で、滋賀県高島市(人口約5万5千人)において循環器疾患の地域悉皆登録および生活習慣病の発症および死亡をアウトカムとするコホート研究を実施している。1988年から2009年4月現在までに悉皆登録において脳卒中2832例、急性心筋梗塞初発例703例を登録した。登録症例のうち1990年から2001年までの症例を用いて1990年代の循環器疾患発症率の推移を観察したところ、出血性脳卒中(脳出血およびくも膜下出血)は依然減少傾向にあるものの脳梗塞は微増傾向にあることが認められ、28日以内の急性期死亡割合は減少していることが認められた。また、急性心筋梗塞は男性で有意な増加傾向を示していることが認められたが、急性期死亡割合に変動は認められなかった。
また、同時に実施している高島市におけるコホート研究では、2008年までに研究協力の同意を得た6093名のうち、血圧脈波検査装置(フォルムPWV/ABI)を用いて動脈硬化指標の計測ができた2480名について死亡と動脈硬化指標との関連を検討した。対象者をbaPWVの値で低値(
