Research Abstract

「研究の目的」大動脈瘤におけるテネイシンCの病態バイオマーカーとしての有用性を実証し、分子機能を解明して治療標的としての有用性を検討することを目的とした。
「研究実施計画」バイオマーカーとしてのTN-C:ステントグラフト治療前後に瘤の形態計測と同時にTN-Cの血中濃度を経時的に計測し、治癒マーカーとしての有用性を検討した。手術切除標本あるいは剖検で得られた大動脈瘤壁の病理組織においてTN-Cの局所発現を解析し、TN-C発現と瘤の進展がどのように関連するかを検討した。更に大動脈瘤病態におけるTN-Cの役割を研究するためにTN-CノックアウトマウスでC aC 12誘起大動脈瘤を作成し、瘤の形態および病態に関わる種々の分子群を定量し、野生型との比較により大動脈瘤におけるTN-Cの役割を検討した。
「研究の成果」
1)腹部・胸部大動脈瘤患者72例の血中TN-C濃度について検討した結果、ステントグラフト治療前では、35～110ng/ml(正常値42+8ng/ml)であり、瘤の大きさとは直接的には関連はなかった。ステントグラフト治療後では、直後には全例において著明に増加し、30日後にはほぼ正常値付近に低下した。血中MMP9値は必ずしも上昇せず動脈瘤の診断バイオマーカーとして有用ではないことが判明し、また血中TN-C濃度との関連はなかった。
2)ヒト大動脈瘤壁のTN-C発現:大動脈瘤の人工血管置換術に際して、患者本人の書面による承諾を得た上で大動脈瘤組織を採取し、免疫組織化学的にTN-C発現を検討した。瘤組織の中で破壊が進行しつつある部位でTN-Cの発現が高く、発現パターンはほぼ平滑筋細胞の分布と一致していた。
3)マウス大動脈瘤モデルにおいて組織TN-C発現は大動脈径が拡大する時期に一致して高値を認め、TN-C遺伝子活性は大動脈瘤壁の血管平滑筋細胞に限局していた。TN-Cは大動脈血管平滑筋細胞の病的状態を反映する新たなバイオマーカになる可能性が示された。
4)C aC 12とアンジオテンシンIIによる複合刺激を加えると大動脈上部に強いTN-C遺伝子活性亢進を認めた。TN-CノックアウトにC aC 12とアンジオテンシンIIの複合刺激を加えると、上部大動脈解離を認めた。このことから、TN-Cは分子的ショックアブソーバーとして働くことが示された。
