| Research Abstract |
Based on the nation-wide survey of transitional care for adolescents with pediatric chronic disease, we developed the transitional care manual for health care worker who support chronically-ill adolescents and their family in addition to the interactive program to support the transitional process for adolescent patients. Continuing education about transitional care and case discussion were also provided for nurses to build the supportive strategies improving psycho-social adaptation of chronically-ill adolescents into adult centered health care system.
