狂犬病迅速診断法の開発と麻痺型狂犬病臨床診断への応用-アジアにおける多施設検討

Research Abstract

狂犬病は主にアジア・アフリカ諸国で流行し、全世界で年間5万5千が死亡していると推定されている。日本人旅行者の死亡例も近年報告されており、日本における再興感染が懸念されている。また、狂犬病には、恐水症や恐風症と言った典型的な症状を呈す狂躁型と、この典型的な症状を経ずに麻痺が起こり、最終的に昏睡に陥る麻痺型狂犬病とがある。この麻痺型狂犬病は、ギランバレー症候群や、他の脳炎との鑑別が非常に困難であり、流行拡大の原因の一つとしても懸念されている。現行の狂犬病の実験室診断法は高価な実験施設や実験機器、さらに高度な技術を必要とされるなどの問題点が挙げられる。
そこで、我々は、(1)先に開発した狂犬病ウイルス抗原迅速イムノクロマト法(Rapid immunochromatography test : RICT)を実際の臨床診断に応用し、その有用性を検討する。(2)RICTを用いた原因不明脳炎、もしくは、急性弛緩性麻痺(AFP)患者における麻痺型狂犬病の有病率を調査する。(3)分子疫学的解析によって、アジア諸国で流行する狂犬病ウイルスの遺伝子型を把握する。以上のことを研究目的とした。
タイ、ブータンおよびスリランカにおいて狂犬病疑いのヒトおよび動物脳検体(計503検体)を用いて、既存の蛍光抗体法(FAT)と比較しRICTの有効性を検討した結果、ヒトにおける検出感度は99%、イヌでは95%、ネコでは74%であった。また、スリランカおよびバングラデシュにおける原因不明脳炎・AFP患者(計271検体)の脳脊髄液から、RICTによって1例の麻痺型狂犬病を同定した。また、Multiplex PCR、16S rRNA系統解析法等により、原因病原体を決定した。さらに、バングラデシュ、スリランカ、ブータンおよびラオスにおける狂犬病の分子疫学解析を行い、その流行状況を明らかにした。
