B型肝炎ウイルススプライシングバリアントによる慢性肝炎の制御

Research Abstract

1「研究の目的」で述べた、肝炎沈静化におけるHBVコア遺伝子のアミノ酸配列に関して検討した。その結果、(1)Basic Core Promoter領域の変異(nt. 1762)が少ないほど肝炎は沈静化しやすい。 (2)肝炎が沈静化しやすい症例でコア遺伝子のアミノ酸置換数がより多い。(3)CD4 epitope領域のアミノ酸置換が起こることが肝炎の沈静化と関連する可能性がある。一方B cell epitope領域のアミノ酸置換が生じないことが肝硬変、肝細胞癌への進展と関連する可能性がある。点が明らかになった。
2研究の中心であるHBV Splicing Variantに関しては、得られたクローンの詳細な解析を行った。core領域末端からPres顔域全体および S領域の一部まで(nt, 2448〜488) (nt. 2448〜2934、3019〜488)のdeletion core領域内(nt. 2088〜2349)にdeletionを有するクローン(2.1kb長)がsplicingvari去nt.の主体であったが、この他に6種類のSplicing Variantが存在した。このうち3種類はコア域のアミノ酸の大きな欠失(67AA, 87AA)を伴っていた。
3 Splicing Variantは無症候性キャリアの47クローン中2クローン(4%) 、慢性肝炎の54クローン中27クローン( 50%)に認められ、慢性肝炎で有意に高率に認められた( p<0.0001 )。
4 経時的な検討を行ったところ、肝炎の発症とともにSplicing Variantが出現し、肝炎の沈静化とともに消退する傾向が認められたsplicing Variantがウイルスの増殖及び肝炎を抑えることを示唆する成績と思われる。
