冠動脈プラーク不安定化における単球及び血管内皮前駆細胞の関与と治療法の確立

Research Abstract

【目的】平成20・21年度分の研究において、末梢血単球のサブセット(CD14+CD16-,CD14+CD16+)の動態は、急性冠症候群における冠動脈プラーク破裂に関連することを明らかにした。
平成22年度においては、(1)安定狭心症患者における不安定プラーク同定と末梢血単球サブセットとの関連、(2)スタチン系薬剤投与による末梢血単球サブセットおよび冠動脈の線維性被膜厚に及ぼす効果について検討した、【方法および結果】(1)61安定狭心症患者を対象に、末梢血単球サブセットの分布様式とmulti-detector computed tomography(MDCT)検査により同定された不安定プラークの関連を検討した。その結果、CD14+CD16+の相対的増加は安定狭心症患者における冠動脈プラークの不安定化と関連することを明らかにした。(2)不安定狭心症患者で急性期に冠動脈形成術(PCI)を施行し、再灌流に成功した40症例を対象に、スタチン系薬剤投与による末梢血単球サブセットおよび冠動脈の線維性被膜厚に及ぼす効果を検討した。その結果、スタチン系薬剤の投与は線CD14+CD16+単球を有意に低下させた。また、線維性被膜厚の変化はCD14+CD16+単球と有意な負の相関関係を認めた。【結論】末梢血単球のサブセットは、急性冠症候群発症前の不安定プラーク同定および薬物治療の効果判定に関して、優れたバイオマーカーになり得る可能性が示唆された。
