Research Abstract

本研究では,申請者らが開発したマルチモーダル視覚刺激を用いた視覚誘発電位(VEP),事象関連電位(ERP),機能的MRI(fMRI)により,健常若年成人,老年成人,軽度認知機能障害(MCI)およびアルツハイマー病(AD)における脳の反応性の違いを検討することで,MCI患者の早期診断とサブタイプ分類をより定量的・客観的に行う方法を確立することを目的としている.具体的には,(1)健常若年成人,老年成人,MCIおよびAD患者おいて,マルチモーダル視覚刺激を用いてVEP,ERP,fMRIを記録する.(2)各群における脳反応(VEP,ERP,fMRI)パターンを比較することで,ADを発症するリスクの高いMCI患者を鑑別できる早期診断マーカーを発見する
九州大学では各群でVEPとERPの記録を行った.MCI患者では各視覚刺激の中で運動視刺激に対するERP反応(下頭頂小葉が発生源)が健常老年成人に比べ最も遅延しており,そのERP反応が最も良い早期診断マーカーであることが示唆された.広島市立総合リハビリテーションセンターでは,運動視刺激を用いたfMRI計測を行った.MCI患者では老年成人でみられた下頭頂小葉の活性化が有意に低下していた.PETやSPECTによる先行研究で下頭頂小葉の代謝・血流の低下は佃に移行するMCI患者のマーカーとして有用であるとの報告があり,これは我々の研究結果を支持する.以上の結果から,運動視刺激を用いたERPやfMRI反応がMCI患者の非侵襲的早期診断法として最も有用である可能性が示唆された
