関節リウマチの多様な病態はCaMKIIδの機能異常でどこまで説明可能か?

Research Abstract

シトルリン化責任酵素であるPADI2、PADI4とCaMKIIとの関連についてはPADI2とPADI4のin vitro実験系の確立について解析した。これら遺伝子導入系に加え、リコンビナントPADIs(rPADIs)(rPADI2、rPADI4)で滑膜線維芽細胞抽出蛋白のin vitroシトルリン化を誘導させることができた。このin vitro無細胞実験系を用い、関節リウマチ患者血清が如何なるシトルリン化分子を認識するか?また、CaMKII(各アイソフォーム)はこの候補分子か?を確認することが可能となった。それに加え新たな系での解析を免疫複合体の網羅的な解析:イムノコンプレキソーム解析で着手した。これは患者血清に自己抗原抗体複合物(イムノコンプレックス)が存在すればそこから抗原を同定する手法であるが、すでにいくつかの候補分子が同定された。この手法からも関節リウマチ患者血清が如何なるシトルリン化分子を認識するか?また、CaMKII(各アイソフォーム)はこの候補分子か?の課題にアプローチすることが可能となった。臨床的な局面からは抗CCP抗体と関連するMRI骨髄浮腫と抗CCP抗体価を計時的に解析した。初診時から抗CCP抗体が陽性の早期関節リウマチ患者を2年間にわたりMRI骨髄浮腫と抗CCP抗体価をフォローした場合、抗CCP抗体価が高いとMRI骨髄浮腫が検出されやすいことが明らかとなった。これはPADIs(PADI2、PADI4)↑→抗CCP抗体産生→関節傷害のプロセスを想定させる。これらサンプルを用いて上記の検討を加えると、シトルリン化およびCaMKII活性化の動的変動と関節傷害との相関が明らかとなる。
