天疱瘡モノクローナル一本鎖抗体を用いた新規の客観的重症度評価法の開発

Research Abstract

天疱瘡の重症度を客観的に測定、評価する方法の開発を目的として、落葉状天疱瘡患者からファージディスプレイ法を用いて単離してきた多数のモノクローナル抗デスモグレイン1抗体の詳細なエピトープ解析を行った。
天疱瘡自己抗原であるデスモグレイン(Dsg)は前駆体Dsg(pro-Dsg)の形で生成され、細胞内でprocessingを受けることにより、細胞間接着機能を持った成熟したDsg(mature-Dsg)となることが知られている。我々は、processingで影響を受けるDsgのエピトープに着目して抗体の反応性を検討し、天疱瘡患者血清中の自己抗体の大部分はProteolytic processingで露呈される部位を認識していることを昨年度の研究実績として報告している。今年度は、天疱瘡患者血清中自己抗体のmature-Dsgおよびpro-Dsgに対する反応性の経時的推移と病勢との相関を検討した。多くの症例において、mature-Dsgに対する反応性がより病勢を反映する傾向が認められた。少数の症例において、pro-Dsgに対する反応性が病勢と相関しない症例が認められた。従って、mature-Dsgのみを抗原としたELISA法はより病勢を反映した検査法になると考えられた。この結果は、International Pemphigus Meeting, (Berne)、The 4^<th> joint meeting of Japanese dermatology association and Australasian college of Dermatologistsを含む国内外の学会で発表し、論文もJournal of Investigative Dermatologyに受理されている。
