重症虚血肢に対するハイブリッド治療-センダイウイルス遺伝子治療と細胞移植療法-

Research Abstract

シロスタゾールには血管新生効果があり、この作用はNOを介した経路によって機能していることが前年度の研究結果より推測された。本年度は重症虚血肢に対するセンダイウィルスを用いた遺伝子治療と自己骨髄細胞移植あるいは末梢血単核球細胞移植療法のハイブリッド治療の完成を目指すための基礎研究としてマウス重症虚血肢モデルにおけるメトプォルミンの血管新生効果の検討を行った。
【方法】 マウス野生型を用いて左大腿動脈およびその分枝を結紮して虚血モデルを作成しメトプォルミン投与群(high dose群(MtH群)、low dose群(MtL群))と通常飼料投与群(Ctr群)で術後3, 7, 14, 21, 28日目にレーザドプラ血流計による下肢血流の評価を行った。
【結果】 レーザードプラ血流計による解析では虚血肢/健肢の平均値は術後3日目でMtH群0.04、MtL群0.14、Ctr群0.14、7日目でMth群0.07、MtL群0.43、Ctr群0.24、であった。以上よりMtL群ではCtr群と比較して虚血状態が改善したと考えられるが、MtH群では改善を認めなかった。
【今後の課題】 今までの標本の筋肉組織サンプルからの研究によりシロスタゾールの血管新生効果にNOが関与していることが判明し、またlow doseメトプェルミンの虚血改善作用効果が示唆された。さらにVEGF、HGF等をはじめとした分子物質の発現に関して検討し、重症虚血肢に対するセンダイウィルスを用いたハイブリット治療の臨床応用へ進展させることが今後の引き続きの課題である。
