Outline of Annual Research Achievements

本研究ではリンパ節介在血行性転移理論の普遍性を検証し, この概念を根拠に臨床的N0センチネルリンパ節のLDDSの局所制御が全身転移の治療にも有効であることを立証するものである. 本年度は以下の2つの課題に取り組んだ. (A)リンパ節介在血行性転移理論の普遍性の検証, (B)リンパ行性薬物送達法(LDDS)の溶媒特性と抗腫瘍効果の検証である.
課題(A)については,ルシフェラーゼ遺伝子および光変換型蛍光タンパク質ベクターを共発現する光変換型腫瘍細胞KM-Luc/pDendraの樹立をおこなった. 光変換型腫瘍細胞のタンパク変換を可能にするレーザーシステムを開発し, レーザー光の出力と蛍光タンパク質ベクタへ光変換効率を求めた. リンパ節介在血行性転移理論がヒトにおいても担保されるかを検証するために, 頭頸部がん患者および乳がん患者から摘出されたリンパ節の組織標本からの3D像を構築し, 血管構造を調べた. 非線形レジストレーションを使用し無理のある変形を軽減した3D像の構築に成功した. 加えて, 音速像および病理像の三次元マップを作成し次元的に細胞の数密度が疎かつ音速値が低値を示す傾向が確認された.
課題(B)については, 抗がん剤であるシスプラチンを使用して, 溶媒の浸透圧, 粘度を変化LDDSの溶媒特性に最適な浸透圧( < 1,900 kPa), 粘度(< 12 mPas)を見出した.

Strategy for Future Research Activity

(A)リンパ節介在血行性転移理論の普遍性の検証
(1)KM-Luc/pDendra細胞をリンパ節に転移させて, リンパ節からこの細胞が血管を介して遠隔転移を来すことを実証する. レーザー光の出力を高めることが, 光変換効率が高まるかを検証する.
(2)3D病理像構築では, 薄切時の試料の変形や染色時の色むら等を減らすなどの組織切片作製工程を改善し, また,血管構造が維持された組織ブロックを使用するなどして, ミクロレベルでのレジストレーションを達成する.
(B)リンパ行性薬物送達法(LDDS)の溶媒特性と抗腫瘍効果の検証
求めた浸透圧域, 粘度域の溶媒を用いて, 転移リンパ節に対するLDDSを実施して,長期的な抗腫瘍効果ならびに生存率を確認する.
