| Outline of Final Research Achievements |
Home care recipients are often provided with a service plan by certified care manager (CM).Currently, the number of home care recipients is increasing due to the terminal stage of cancer.Our 2019 survey results have clarified cooperation issues of End-of-Life care between medical personnel and certified CM.The problem was that it was difficult to grasp the physical changes of the subject and it was not possible to properly provide home care services during the stage of the disease.Therefore, we considered a proposal for practical training for CM (Visiting home care recipients with medical personnel)Requests for training content included “progress during the disease stage”, “response to sudden changes”,” consideration for family members”, and “exchange of opinions with doctors”.Summarizing the results so far, we drafted a program for practical training with home-visiting nurses.

| Academic Significance and Societal Importance of the Research Achievements |
介護支援専門員（以下CM）の医療職との連携を踏まえたCMの看取りの実務研修教育プログラムの立案は、地域包括ケアシステムを遂行していく課程で、対象者が可能な限り住み慣れた地域で生活を継続することができるよう医療と介護の連携，医療と介護の提供体制を一体的に整備するため、CMが、在宅医療と、介護保険を利用している利用者へのケアプラン作成には、患者の状態を収集し家族との関係や、思いも含めアセスメントしていく能力が育成され、CMとして医療面ではなく、介護面での支援ができるようになる実務研修プログラムの立案は在宅における看取りに意義がある。
