Outline of Final Research Achievements

This study was aimed to develop Advance Care Planning (ACP) e-Portfolio, which uses a qualitative analysis software. We developed an educational program for healthcare professionals who support dignity of older people, and conducted such programs targeting 66 healthcare professionals. We are currently analyzing the data obtained before and after the intervention: ACP discussion and documentation rate and content.

Academic Significance and Societal Importance of the Research Achievements

ACPには、医療・介護・福祉の多職種が関わる必要がある。実際、米国を中心に、2015年以降ソーシャルワーカーのACPへの関与を期待する論文発表が増えているが、日本におけるACPの普及活動では、延命・救命処置の希望に関する事項（リビング・ウィル）が中心に据えられ、本人の価値観、人生観、死生観、ライフレビューなど「その人らしさ」に関わる部分にはあまり焦点が当てられてこなかった。こうした尊厳の根幹に関わる内容もACPの重要な項目の一つとして関係者により深く認識されるべきである。
