| Outline of Annual Research Achievements |
最終目的は、独自開発のゲノム診断法で画期的な術後化学療法プログラムを開発することにある。我々は血中微量遊離がん由来DNA(ctDNA)に注目している。しかし過去の研究結果より周術期の薬物療法は進行癌の薬物療法と全く異なる考え方をしなければならないという考えに至っている。このため、特定の体細胞変異に加え、DNAメチル化及びDNA断片化といったがんエピゲノム異常を評価することで高い検出感度で微量残像病変（MRD）を評価することを考えている。 Guardant RevealアッセイはGuardant Health社が開発した新規ctDNA MRDアッセイで、デジタルシークエンシング技術を用いて、特定の体細胞変異に加え、DNAメチル化及びDNA断片化といったがんエピゲノム異常を評価することで高い検出感度でMRDを評価することを目的としている。我々はGuardant社と協力し、予備的検討を行い、進行大腸癌では、84％の陽性率であることを確認した。そこでこの手法を用いて、切除可能遠隔転移大腸癌の、切除後の再発を検出するプログラムを開発することを考えている。

| Strategy for Future Research Activity |
我々は、今後Guardant社と協力し、切除可能転移性大腸癌（OM-CRC）を対象にGuardant Revealアッセイ(ctDNA+エピゲノム)を用いて治癒切除後OM-CRCの再発リスクを判定し、術後ctDNA陽性症例に対しては、mFOLFOX6療法を比較対照として、FOLFOXIRI+BV療法の有効性と安全性を多施設共同臨床ランダム化第II相試験で評価する予定である。
