口腔がんと関連するマイクロバイオームとそのリスクを反映するエピゲノム異常の同定

Outline of Annual Research Achievements

口腔がんの素地の形成（発がんのリスク）・病態に関与するオーラルマイクロバイオームの探索・同定を試みている。口腔内細菌と口腔がんとの関わりが見えにくい理由として、喫煙や飲酒の発がんへの影響が非常に強く、口腔内細菌の影響が見えにくくなっていることが一因と推測される。そこで、本研究においては、口腔内細菌と喫煙や飲酒による影響を排除するため、対象患者を非喫煙かつ非飲酒の者に絞って解析・検討を行う。口腔がん患者（30例）ではがん同側の非がん歯肉の歯周ポケットから、健常者（30例）では臼歯部の歯周ポケットから口腔内細菌を採取することとした。現在は、その前段階として、研究申請者、分担研究者において口腔内細菌採取、同定を試みている。ワッテなどで唾液を遮断し、綿球にて採取部位周辺のプラークを取り除き、滅菌済みのペーパーポイントを歯周ポケットに挿入し、歯周ポケット内の細菌の採取を試みた。採取した細菌からrRNAを抽出し、次世代シークエンサーを用いて口腔内細菌約４００種類 [歯周炎と強い関連があるRed Complex（Porphyromonas gingivalis, Treponema denticola, Tannerella forsythensis）やRed Complexではないが歯周病やう蝕に関連する菌種（Actinomyces actinomycetemcomitans, Prevotera intermedia, Fusobacterium nucleatum, Streptococcus mutans, Streptococcus sobrinus）や全身疾患との関連が報告されている菌種（Streptococcus milleri group, Streptococcus mitis, Streptococcus oralis）など]の割合の測定をトライしている。

Reason

コロナウイルスの感染拡大の影響を受けた。口腔内からの検体採取は、感染リスクが高く、研究を進めにくい状況であった。

Strategy for Future Research Activity

口腔がんの発症・病態に関与するオーラルマイクロバイオームの探索・同定を進める。それが終了したら、口腔粘膜における発がんリスクを反映するDNAメチル化異常の探索・同定を行う。

Causes of Carryover

コロナウイルスの感染拡大により研究に遅延が生じた。
