Outline of Annual Research Achievements

大豆・魚の蛋白質が食塩負荷した脳卒中易発症ラット(SHRSP)の脳卒中予防に有効であった実験成果をヒトで検証するため,世界保健機関(WHO)の共同研究を提唱し,1985年より世界の61地域で24時間尿を採取し,大豆と魚の摂取が心筋梗塞の年齢調整死亡率と有意の逆相関を示すことを検証した(2017).
この成果を日本人で検証するため兵庫県の健診で大豆イソフラボン(Iso),魚のタウリン(T)摂取の最も多い群は,少ない群に比べ,認知症の予防効果が期待される血中葉酸値と動脈硬化を抑えるHDLコレステロールが有意に高いが,食塩の摂取が1日4~5gも多く,高血圧から脳卒中が多く,健康寿命は平均寿命よりも10年も短い原因と分かった.
そこで,適塩での大豆・魚の摂取が健康寿命の延伸に寄与するか否かを検証するため,①長浜市の住民13,000人を対象とする「長浜コホート研究」と,②佐賀県脊振町住民健診で60歳以上の人で健診結果を分析した.すでに脳機能検査をした後者の集団,475名中296名につき24時間尿中ナトリウム(.Na),カリウム(K),マグネシウム(Mg),大豆Iso,などを分析した.特に,大豆・魚の摂取の多い群で葉酸(F)の高かった兵庫県民のデータから,Fの増加とHomocysteine(H)が逆相関することから,血中Fと尿中Hには弱い正相関を検証した.以上の成果は大豆・魚の適塩摂取でFの上昇を介して血中Hを減少させHによる酸化作用の抑制から動脈硬化や認知症の予防に貢献する可能性がある.
①の長浜研究については,COVID-19の世界的流行のため2017年以来実施してきた採尿健診は中止したが,2022年には再開し,健診対象者7619人中,採尿実施者は1855人,そのうちの採尿健診参加者で認知症機能検査(受診者概数1569人)のデータが共有され次第,統計分析を進め,認知症予防の栄養因子の解明に貢献する.
