Outline of Final Research Achievements |
Early post-operative maxillofacial rehabilitation is effective for QOL and social activity of patients have maxillofacial defects after surgical operation. Though immediate surgical obturation for maxillary defects is adopted to patients as prosthetic standard therapy, prosthetic rehabilitation for mandibular defects differs from institutions. Also there is no clear treatment goal for mandibular reconstruction. Use of guide-splints enables restorative-driven approach for mandibular reconstruction. It makes our treatment goals clear and maxillofacial rehabilitation using prostheses easier. We adopted immediate dentures as guide-splints and treatment dentures to mandibular reconstruction. Immediate denture adaptation needs appropriate designs of denture flange and fibula placement in a mandibular defect. It has a potential for wide adaptation to patients.

Academic Significance and Societal Importance of the Research Achievements |
外科的再建時に使用するガイドシーネは術前の歯列形態や顎間関係を参考に製作することからrestorative-drivenな顎骨再建を可能とする。ガイドシーネの利用は症例に応じた再建顎骨形態を明示することで術者間での治療目標を明確にすることが出来る。 即時義歯の利用は、術前腓骨配置シミュレーションおよび術中の皮弁移植時のOcclusal-drivenな顎骨再建に有用であり、術後は、治療用義歯として下顎位の安定化に寄与するなど術後早期からの口腔リハビリテーションにも効果を認めた。義歯および再建顎骨形態に配慮した下顎再建および即時義歯の使用は、無歯顎患者を含めた広範囲な患者への適応が期待できる。
