網羅的ゲノム・エピゲノム解析による小児固形腫瘍の分子機構解析と分子標的療法の開発

Research Abstract

これまでにSNPアレイ解析から神経芽腫200例からALK遺伝子を(Nature 455:271,2008)、リンパ腫から6番長腕のuniparental disomy (UPD)からA20を、骨髄増殖性疾患の11番のUPDからCBL遺伝子を同定した(Nature 459:712-716,2009)。この手法を用いて、小児の固形腫瘍のUPD領域からゲノムインプリンティングの網羅的な探察を行った。
今年度は、ユーイング肉腫(Ewing sarcoma family of tumors, ESFT)99検体(細胞株17株を含む)を用い、ALK遺伝子の変異解析を行ったところ、新規のミスセンス変異が4例に認められた。その一部について機能解析を行ったところ、ウェスタンブロットにおける自己リン酸化の亢進、siRNAによるALK knockdownでの細胞増殖の抑制、コロニーアッセイにおいて足場非依存性増殖、ヌードマウスアッセイにて腫瘍形成が観察された。また、ALK阻害剤の投与にて、ALK変異を持つESFT細胞株においても高い効果を認めた。神経芽腫のみでなく、一部ESFTにおいてもALKが関与することや治療の標的になることが示唆された。ALKの発現はESFT46例全例で確認され、ESFTの中では極めて稀な鼻腔原発の2例にkinase domainの胚細胞変異(活性型)を確認した。
横紋筋肉腫(RMS)50検体(細胞株8株、新鮮腫瘍42例)の解析では、RT-PCRを施行した29検体中17検体(59%)でALKの高発現を確認し、SNPアレイでは新鮮腫瘍1検体(胞巣型)でALK領域の高度増幅を検出した。ALK阻害剤により一部細胞株で増殖抑制効果が認められた。ALKはRMSの発症・進展に関与している可能性が示唆された。
