自己免疫性肺胞蛋白症の国際疫学研究と診断治療国際的標準化:稀少肺疾患研究基盤

Research Abstract

肺胞蛋白症(PAP)に対する国際共同多施設横断的調査研究(MICEPAP):米国稀少肺疾患コンソーシアムと共同で日米独伊におけるautoimmune PAPの多施設国際横断的評価を行った。健常コントロールを含む147名の登録を終了し、更に血清中抗GM-CSF自己抗体を我々の標準法で中央測定し比較した。人種差は認めなかった。国際肺胞PAP会議で発表した。
(2)抗GM-CSF抗体の測定の国際的標準化:MICEPAP等を通じてautoimmune PAP診断の要である抗GM-CSF抗体の測定の標準化を行った。日本(近畿中央胸部疾患センター、新潟大学)米国(シンシナテイーこども病院医療センター)の3施設の検査室での同一検体の測定データを比較した。良い相関が得られ、標準抗体を用い国際単位(IU)設定を検討するに至った。
(3)多国間のautoimmune PAP臨床データの比較:独と日本の比較を実施。欧州連合PAP会議、国際肺胞蛋白症会議にて発表した。粉塵吸入喫煙の影響が強調された。アジアの国との比較も検討課題となった。
(3)PAPの国際的ガイドライン:米国胸部疾患学会、稀少肺疾患コンソーシアムと共同でPAPのガイドライン作成の会議のまとめが進行中。他研究費のよる我が国の指針の一部英訳を海外へ紹介した。
(4)平成23年3月29日～31日、欧州連合PAP会議、国際肺胞蛋白症会議(伊パビア)参加発表。日本のPAP研究グループと欧州PAPグループの継続的連携が確認された。平成23年5月13日18日、米国(デンバー)にて国際コンサンサス、ガイドライン作成のための会議参加。国際共同研究体制が構築されつつある。国際稀少肺疾患共同研究基盤により稀少肺疾患リンパ脈管筋腫症の研究が完遂された。
